Insurance Information

Currently, we are accepting the following insurance plans: Sloans Lake, Aetna, Great West, PacifiCare, Blue Cross Blue Shield, PHCS, United Health Care, MediCare and Health Savings Accounts.

If we are not a provider of your individual insurance or are self-pay, payment can be made by personal check, Visa/Mastercard or cash at the time of your visit.  We will provide you with a bill which contains all information required from us for you to submit to your insurance carrier.

Office Hours

We see patients from 9:00AM to 4:00PM on Tuesday's and Thursday's.  Our office staff is available Monday thru Friday from 8:30AM to 4:00PM. For any inquires or to schedule an appointment, please call (970) 845-7872.

If you are unable to reach us during normal business hours, please leave a voice message and we will return your call promptly.
